Prescription for Health is a five-year initiative funded by The Robert Wood Johnson Foundation (RWJF) in collaboration with the Agency for Healthcare Research and Quality (AHRQ).

Under this initiative, primary care practice-based research networks (PBRNs) are developing creative, practical strategies for promoting healthy behaviors among patients and transforming primary care practice. The four leading health risk behaviors are targeted:

  • Lack of physical activity
  • Unhealthy diet
  • Tobacco use
  • Risky use of alcohol
